Testing for the coronavirus (COVID-19)



Testing for the coronavirus (COVID-19) has varied widely across countries. To reduce the risk of new outbreaks, countries will need to greatly increase their testing capacity. 


A  key  question  behind  any  strategy  to  ease confinement  restrictions  and  reopen  economic activities is how to avoid a new spread of the SARS-CoV-2 virus that would necessitate further lock downs.  Once the  number  of  infected  people  has  successfully  been  brought  sufficiently down, quick  suppression  of new  waves  of  viral  infections  will  be  key.  Testing  strategies  are central to achieve this.

There are two types of tests.

First, molecular diagnostic testing (RT-PCR) helps to identify those individuals who are infected at the time of the test. An effective strategy that tests, tracks people infected and traces their contacts (TTT), helps to reduce the spread of the virus and thus bring its reproduction number below one.
Given the characteristics of this coronavirus–including the large number of asymptomatic cases and high reproduction number –to be effective at suppressing the spread of the virus, the TTT strategy should be used very widely, requiring a very large proportion of all cases (between 70 and  90%) to be traced to  prevent  a new outbreak of the  virus. This  would require increasing capacity for testing enormously; putting in places trict measures to prevent people who may be infectious from breaking quarantine; as well as identifying ways to trace contacts,which may push the limits of privacy concerns, unless new approaches to digital tracing, currently under development, are put in place.
Significant logistics and capacity constraints –ranging from the availability of trained personnel to  take  accurate  specimen,  to  the  time  required  for  laboratory  analysis  and  the  availability  of reagents –have impeded more widespread diagnostic testing in many countries so far. Recent development of faster RT-PCR molecular diagnostic testing, which can be deployed at the point of care, should help scale-up capacity for  effective TTT in countries. Digital  enabled contact-tracing can  help  improve  the  speed  and  effectiveness  of  TTT  strategies,  as  seen  in  some countries.

A second type of test –so-called serologic test –detects people who have had a prior infection and  thus  developed  antibodies.  Such  tests  can  be  used  for  two  purposes,  namely  to  allow people who have acquired immunity to return to work safely, and to provide intelligence on the evolution of the epidemic across the population. Rapid serology test kits need to be developed and  their  clinical  performance  needs  to  be  demonstrated  before  deployment  at  scale  can happen.
Despite the fact that a relatively low number of people have so far been infected and thus we are still far from herd immunity, the successful implementation of serologic testing strategies at large scale can help reduce the spread of the virus and complement the TTT strategy.

This will also require major efforts, including: 

1) verifying the clinical performance of tests, particularly for rapid  serologic  tests; 

2) preparing  procurement  and  logistics  arrangements  to  scale  up production and deployment of the tests, and train and deploy human resources, particularly for diagnostic RT-PCR tests; and

3) providing adequate safeguards to protect civil right and privacy of populations while deploying or apps-enabled tracking strategies.

Since the end of last year, the world has been in the grip of the SARS-CoV-2 virus, which has caused tens of thousands of deaths from the respiratory disease COVID-19. To combat the pandemic, many countries have put in place strict containment and mitigation strategies to minimise the risk of transmission, decrease the spread of the virus and ‘buy time’ for health care systems to cope with the huge numbers of patients and ultimately save as many lives as possible. As part of the response to COVID-19, virtually all OECD countries affected by the virus have introduced strict restrictions to social and economic life, including social distancing and even full lock downs. The big question  is  now  how  to  manage  these  restrictions,  and  how  to  go  back  to  a new  normal of  living  with SARS-CoV-2; a social and economic life that coexist with the virus. To avoid new peaks in the number of cases, overstretching health system capacities, infection rates need to remain suppressed until a vaccine or  effective  treatment  are  found.  If  all  confinement  strategies  are  lifted,however,  the  infection  rate  is expected to rebound in a matter of weeks. A strategy is needed about when and how to relax confinement, and when and how to re-tighten some of them when necessary. This is needed to minimise the risk of further peaks of the outbreak or, at least, to win as much time as possible between the successive peaks.

A number of factors need to be in place to achieve this goal. 

First, healthcare capacity and resources need to be increased to ensure safe and effective management of future severe COVID-19 cases.

Second, we need to understand the virus better, including: the incubation period and infectiousness of the disease  at  different  stages;  the  extent  of  asymptomatic  spread;  immunity  and  its  duration  in  those  who contracted the virus; and the impact of changes in temperature on the disease spread.

Third –and the topic of this brief –information about the presence and propagation of SARS-CoV-2 in the population  needs  to  improve  significantly. For  this,  widespread  testing  and  effective  contact  tracing, including cases with no or only mild symptoms, are key components of the post-lock down strategy. Better information will help achieve three goals:

Tracking of new cases to suppress the resurgence of local outbreaks as early as possible, aiming to avoid new peaks;

Identifying previously infected people who can safely return to work, to revitalise the economy and to strengthen the health workforce;

Gaining  intelligence  on  the  evolution  of  the  epidemic,  including  on  when a  threshold  for herd immunity has been reached.In the case of COVID-19, it has been estimated that 50% to 60% of the population needs to be immune to the virus to halt its spread.

App Immuni by Bending Spoons S.p.a. (Il testo è in italiano)




Italija je odobrila aplikaciju koja će pomoći u praćenju infekcija u fazi 2. To je aplikacija Immuni tvrtke Bending Spoons S.p.a. iz Milana. Ovakva tehnologija uskoro se primjenjuje i u ostatku svijeta pa tako i u Republici Hrvatskoj.

Primjena novih tehnologija omogućava praćenje blizine mnogo učinkovitije i brže od tradicionalnih što nije uvijek učinkovito i uključuje veće trošenje resursa. Aplikacija treba omogućiti upravo to: pronađite ljude s kojima je pozitivni subjekt Covid-19 došao u kontakt prethodnih dana i rekonstruirati povijest njegovih pokreta.

Južna Koreja često se navodi kao model, kao i Singapur, gdje su usvojena rješenja slična onome koje je sada izabrala Italija. Sustavi praćenja koji se koriste u tim zemljama rezultat je procesa koji je započeo prethodnim epidemijama poput SARS-a i MERS-a.

Kada je Pandorina kutija otvorena, iz nje su, među ljude koji su do tada bili živjeli bezbrižno u blagostanju, izašla sva zla: kuga, tuga, siromaštvo, zločin, očaj, pohlepa, starost, bolest, ludost, prkos, glad, prijevara... Pandora ju je brzo zatvorila i unutra je ostala samo nada. Svijet je bio vrlo hladan i turoban, a onda je Pandora opet otvorila kutiju te je nada izletjela van. Tako je čovječanstvo i u zlim vremenima dobilo malo nade.

Druga verzija mita govori da je nada bila zapravo najgore od svih zala jer je bila jednaka strašnom predznanju. Pandora je tako, zatvorivši kutiju prije nego što je nada izašla van, spasila svijet od najgore štete. Naime, sama bi se riječ „nada“ (grč. Ελπις) bolje mogla prevesti kao „predviđanje nesreće“. Mit o otvaranju Pandorine kutije, zapravo je, teodicejsko pitanje: zašto postoji zlo u svijetu?

Dopo aver conosciuto Immuni, abbiamo chiesto ad autorevoli esperti di protezione dei dati, di diritto d’autore e di sicurezza informatica il giudizio sull’app di contact tracing scelta dal Commissario straordinario per l’emergenza Covid-19.

L'Italia ha scelto l'app che dovrà aiutare il tracciamento dei contagi nella fase 2. Si tratta di Immuni, soluzione nata da una sinergia tra il Centro medico Santagostino e Bending Spoons, impresa italiana di sviluppo di app per iOS.  Che cos'è il contact tracing, ossia la tracciatura dei contatti È una delle "azioni di sanità pubblica utilizzate per la prevenzione e il contenimento di molte malattie infettive perché può aiutare a identificare individui potenzialmente infetti prima che emergano sintomi e può impedire la trasmissione successiva dei casi secondari", si legge nell'ordinanza con cui il commissario straordinario Domenico Arcuri ha disposto la stipula del contratto di concessione gratuita dell'app da parte della milanese Bending Spoons. L'importanza del controllo accurato dei contagi è stata sottolineata anche dal direttore generale dell’Organizzazione mondiale della sanità, Tedros Adhanom Ghebreyesus, che a marzo, nel pieno dell'epidemia nel nostro Paese, indicava la strada per interrompere la catena del virus: testare tutti gli individui con sospetta infezione da coronavirus, isolarli se positivi, tracciarne i contatti avuti sino ai due giorni precedenti ai sintomi ed estendere il test anche a queste persone. 

L'applicazione delle nuove tecnologie consente un tracciamento di prossimità molto più efficiente e rapido del tradizionale che non sempre è efficace e comporta maggior dispendio di risorse. L'app proposta da Bending Spoons e Centro medico Santagostino dovrebbe consentire di fare proprio questo: rintracciare le persone con cui è entrato in contatto il soggetto positivo al Covid-19 nei giorni precedenti e ricostruire la cronologia dei suoi spostamenti. Ma come conciliare tutto questo con la privacy? Anonimato, bluetooth e volontarietà In osservanza delle linee guida sulla tutela dei dati personali adottate a livello europeo, Immuni: - garantirà l'anonimato - non utilizzerà la geolocalizzazione (come avviene in Corea del Sud, per esempio) - sfrutterà la connettività Bluetooth - avrà base di utilizzo volontaria L'app in sostanza dovrebbe consentire agli utenti di tenere un forte controllo sui propri dati. I contatti avuti con gli altri vengono tracciati, ma restano 'bloccati' nello smartphone dell'utente fino a quando non si ha certezza che la persona che l'ha installata sul proprio cellulare è risultata positiva al test del Covid-19. A quel punto la persona stessa può dare il consenso al trattamento dei propri dati conservati sul cellulare, permettendo quindi di rintracciare tutti quelli con cui è entrata in contatto nei giorni precedenti. 

Diario e tracciamento L'app, da quanto si è appreso finora, si compone di due parti: un registro sullo stato di salute della persona e della sua eventuale sintomatologia, se affetta da coronavirus, e un tracciamento dei contatti che consentirà al software di riconoscere e tenere memoria dei dispositivi con cui lo smartphone del paziente è entrato in contatto. Nessuno dei dati raccolti verrà raccolto o diffuso prima che il paziente, se affetto da covid-19, abbia deciso di dare il consenso al loro utilizzo.  Il diario clinico È un registro sullo stato di salute della persona. Vi sono indicati i dati che l'utente sceglie di inserire (un po' come per le app di salute o fitness): sesso, età, malattie pregresse, farmaci assunti. Bisognerà aggiornare quotidianamente il diario con eventuali sintomi e dettagli sullo stato di salute. Come avviene anche per chi usa l'app AllertaLOM (CercaCovid) della Regione Lombardia. Tracciamento dei contatti via Bluetooth: Il Bluetooth consente di rilevare la vicinanza di due smartphone (bisognerà capire quali sono i parametri di distanza che saranno adottati). L'utente potrà quindi sapere se è venuto in contatto con una persona positiva al COVID-19. Sarà inoltre possibile ripercorrere tutti gli incontri di una persona positiva per tracciare e isolare i potenziali contagiati: l'app conserva infatti un registro con i codici identificativi anonimi di tutti gli altri smartphone in prossimità dei quali si è stati vicino.




Bluetooth o Gps? Al di là degli aspetti legati alla privacy, la tecnologia bluetooth sembra costituire un sistema più performante in materia di contact tracing. Il bluetooth infatti certifica che due telefoni si sono incontrati in maniera diretta e immediata. Con i sistemi di geolocalizzazione, che comunque possono avere un margine di errore di qualche metro, si colloca una persona in una certa posizione. Per certificare chi era vicino occorre confrontare le posizioni rilevate dal satellite.  Ambito di applicazione dell'app Immuni sarà un'app adottata a livello nazionale: non dovrebbe esserci o quantomeno dovrebbe essere ridotta la frammentazione degli strumenti di tracciamento e monitoraggio che le Regioni stanno adottando in ordine sparso (come la Regione Lombardia). Si dovrebbe partire con alcune regioni pilota - ancora da definire - per poi estenderne l'uso a un'area più vasta. I limiti Il limite principale è lo stesso riscontrato in tutto il mondo: la volontà dell'utente. Per essere efficace Immuni dovrà essere utilizzata su base giornaliera dal 60 % degli italiani.  Dubbi sull'efficacia delle app La discussione è aperta. E non solo per questioni di privacy. La Corea del Sud è stata spesso citata come modello, così come Singapore, dove sono state adottate soluzioni simili a quella scelta dall'Italia. I sistemi di monitoraggio utilizzati in questi paesi sono il frutto di un percorso iniziato per fronteggiare precedenti epidemie come la SARS e la MERS. E quindi possiamo dire, che, in parte, è già stato rodato. Si tratta spesso di Paesi i cui governi godono di un certo livello di discrezionalità in tema di sfruttamento dei dati. In Italia la mappa dei nostri spostamenti è ricostruibile usando solo i dati delle compagnie telefoniche e quelli provenienti da Google, Facebook e Apple.

Per quanto riguarda la privacy, disciplinata in Europa dal GDPR, bisogna ricordare che il decreto-legge del 9 marzo per il potenziamento del Servizio sanitario nazionale prevede, all’articolo 14, in via eccezionale e per la durata dell’emergenza COVID-19, una deroga al GDPR per il trattamento e la comunicazione da parte di soggetti qualificati dei dati sanitari dei cittadini interessati dal Coronavirus. Sul punto si è espresso il Comitato Europeo per la Protezione dei dati (EDPB) che, in una nota diramata il 19 marzo dal titolo Dichiarazione sul trattamento dei dati personali nel contesto dell’epidemia di COVID-19, ha specificato che "Ie autorità pubbliche dovrebbero innanzitutto cercare di trattare i dati relativi all'ubicazione in modo anonimo (ossia, trattare dati in forma aggregata e tale da non consentire la successiva re-identificazione delle persone), il che potrebbe permettere di generare analisi sulla concentrazione di dispositivi mobili in un determinato luogo ("cartografia"). Il caso Singapore Una delle applicazioni in uso è TraceTogether: consente alle autorità locali di rintracciare rapidamente le persone che sono state esposte a casi confermati di coronavirus e di identificare le persone che si sono trovate nelle immediate vicinanze - nel raggio di 2 metri per almeno 30 minuti - di pazienti affetti da coronavirus utilizzando la tecnologia Bluetooth. Un aspetto che, a detta del Government Technology Agency (GovTech) e del Ministero della Salute (MOH), può rivelarsi particolarmente utile nei casi in cui le persone infette non conoscono tutti coloro con cui sono stati nelle vicinanze per un periodo di tempo prolungato.

"Credo che proprio Singapore sia un esempio perfetto per anticipare quello che potrà succedere in Italia, perché lì non è stato deciso il lockdown ma solo alcune restrizioni e l'app come aiuto per il tracciamento dei contagi", spiega all'AGI Marco Trombetti, fondatore di Pi-Campus e tra i maggiori esperti in Italia di nuove tecnologie. Quindi Singapore vive una situazione simile a quella che possiamo possiamo immaginare in Italia con la Fase 2 dell'epidemia.  "E se guardiamo quello che è successo a Singapore, possiamo dire che è stato un disastro", commenta. "L'applicazione adottata dal governo è stata scaricata meno di un milione di volte su circa sei milioni di abitanti, circa il 18%. Di questi solo il 50% ha attivato l'app, quindi circa mezzo milione di persone. Non solo. C'e' un gap nei dati raccolti perché il Bluetooth non traccia automaticamente gli iPhone, che a Singapore sono circa il 38% degli smartphone usati".




Il parere degli esperti: "In chiaro il codice sorgente" "Il codice sorgente dell'app Immuni dovrebbe essere messo in chiaro per trasparenza e possibilità di integrazione" da altri sviluppatori: è il parere di Stefano Zanero, professore associato di computer security al Politecnico di Milano. "L'open source - sottolinea - è una scelta fatta da altri sistemi di tracciamento in Europa e anche da Singapore".   "Rilasciare il codice sorgente serve a poter analizzare l'app in ottica di sicurezza e privacy, per vedere esattamente l'applicazione cosa fa dietro le quinte, oltre che per scoprire eventuali vulnerabilità", dice Andrea Zapparoli Manzoni, esperto internazionale di cyber security governativa."Poiché Bending Spoons - continua - ha dato codice sorgente e proprietà intellettuale dell'app al governo, dovrebbe essere quest'ultimo a rilasciarlo".    "Non è facile dare un giudizio su questa applicazione perché non c'è ancora niente di pubblico, un white paper, un documento che spieghi le caratteristiche tecniche e neanche le osservazioni della task force del ministero dell'Innovazione che ha scelto l'app - osserva Zanero - Ci sono vari modi per fare 'contact tracing' nel rispetto delle privacy e con l'uso del bluetooth e sembra che l'app scelta vada in questa direzione, ma nessuna considerazione è stata resa pubblica, sarebbe utile per valutarla meglio e per capire perché la scelta è caduta su questa proposta e non su altre arrivate alla task force del ministero, come l'altra app considerata in pole position cioè l'app Covid Community Alert che è open source".

La carica delle app Sono almeno una sessantina le iniziative avviate su scala regionale e anche di singola Asl in campo digitale nell'ambito del contrasto al coronavirus, secondo il censimento realizzato nel Terzo Instant Report Covid-19 da Altems, Alta Scuola di Economia e Management dei Sistemi Sanitari dell'Università Cattolica.  Sul fronte "digital" in particolare la Regione Lazio ha disposto l'attivazione dell'app "DoctorCovid", che permette a medici di famiglia e pediatri di libera scelta di gestire le persone in quarantena a domicilio oltre che a monitorare cittadini a casa con sintomi ma che non avevano ricevuto il tampone, contribuendo così decongestionare iCovid Hospital attivati. Le altre Regioni si caratterizzano per diverse iniziative lanciate a livello aziendale, 21 globalmente nelle 6 regioni mappate delle 64 individuate a livello nazionale(tutte le Regioni). Di queste ben 8 sono state attivate da aziende sanitarie della Regione Lombardia e 8 da aziende della Regione Veneto. In Lombardia è attiva l'applicazione AllertaLOM della Regione che ha l'obbiettivo di tracciare la mappa del contagio. Nel messaggio arrivato via sms ai residenti si chiede di compilare ogni giorno il questionario anonimo sul proprio stato di salute che è contenuto nell'applicazione per "aiutare a tracciare la mappa del contagio". All'iniziativa hanno aderito quasi un milione di lombardi. "Grazie all'analisi di questi dati statistici - ha detto il vicepresidente della Regione Fabrizio Sala - riferiti alla più vasta area che subito il maggior contagio da coronavirus, i nostri ricercatori potranno definire una mappa del rischio". Sala ha ricordato: "Il questionario è assolutamente anonimo, va ripetuto ogni giorno e invito chi ha già scaricato la nostra app gratuita dalle piattaforme Ios e Android a invitare le persone che conoscono, i figli e i loro contatti a coinvolgere quanti più cittadini possibili in modo da raggiungere in tempi brevi il nostro obiettivo di un milione di lombardi che dispongono dell'app, partecipano al nostro progetto e aggiornano quotidianamente i loro dati". 

All'incrocio tra monitoraggio e sovranità digitale, considero il problema più grave a monte. In altre parole, anche se ogni nazione rintracciasse i propri cittadini, invece di avere la soluzione paneuropea sperata dalla Commissione, non sarebbe ancora meglio, perché il monitoraggio digitale dei contatti è intrinsecamente pericoloso.

Vaše liječenje bolesti COVID -19 trenutno dostupnim lijekovima u Republici Hrvatskoj, ISKLJUČIVO uz suglasnost nadležnog bolničkog povjerenstva za lijekove i Vaš pristanak.

Liječenje se za sada temelji na liječenju simptoma i potpornom liječenju, te mjerama intenzivnog liječenja za teško oboljele. Istraživanja o pokušajima antivirusnog liječenja COVID-19 su malobrojna te za sada ne postoje dokazi o učinkovitom antivirusnom liječenju utemeljeni na kliničkim istraživanjima.

Unatoč tome, prema prvim do sada objavljenim iskustvima pokazalo se u pojedinim skupinama bolesnika da neki antivirusni lijekovi pokazuju određeno pozitivno djelovanje, iako za sada nisu odobreni za primjenu u liječenju COVID-19 te se radi o primjeni lijekova koji nisu registrirani za liječenje COVID-19.

Za liječenje bolesti COVID -19 nužno su u obzir uzete moguće kontraindikacije u primjeni, nuspojave navedenih lijekova (Resochin®) i interakcije koje može imati s drugim primijenjenim lijekovima, kao i eventualna korekcija doze lijekova ovisno o mogućem zatajenju pojedinih organa i organskih sustava.

U slučaju kliničke sumnje ili dokazane pridružene infekcije (influenca (gripa), bakterijska infekcija), ovom obliku liječenja dodaju se i drugi antivirusni/antibakterijski (Sumamed®) lijekovi. Primjena oseltamivira (Tamiflu®) indicirana je samo u slučaju istovremeno dokazane infekcije virusom influence (gripe). Za sada nema dokaza niti indikacije za istovremenu primjenu više lijekova (kombinacije) za liječenje COVID-19.

Resochin®

proizvod koji je Bayer otkrio 1934. predviđen za prevenciju i liječenje malarije, također ima i antivirusna svojstva širokog spektra te dobar učinak na imunološki sustav tijela. Novi podaci iz početnih pretkliničkih i evoluirajućih kliničkih istraživanja provedenih u Kini, iako su ograničeni, pokazuju potencijal za uporabu Resochina u liječenju pacijenata s infekcijom COVID-19.

Resochin® , a product discovered by Bayer in 1934 and indicated for prevention and treatment of malaria, also appears to have broad spectrum antiviral properties and effects on the body’s immune response. New data from initial preclinical and evolving clinical research conducted in China, while limited, shows potential for the use of Resochin®  in treating patients with COVID-19 infection.


Sumamed®

Skupina istraživača-znanstvenika iz PLIVE stvorila je 1981. godine novi antibiotik pod nazivom azitromicin, po svemu različit od ostalih do tada primjenjivanih antibiotika. U rujnu 1988. godine registriran je PLIVIN originalni antibiotik, azitromicin, poznatiji pod zaštićenim imenom
Sumamed®. Osobine azitromicina od samih početaka upućivale su na to da se radi po svemu jedinstvenom antibiotiku koji ima izvrsnu učinkovitost na najčešće uzročnike infekcija, ciljano djelovanje na mjestu infekcije, vrlo se jednostavno i kratko primjenjuje i ima odličnu podnošljivost. Iz farmakoloških osobina i kliničkih istraživanja proizišle su i originalne marketinške kampanje koje su mijenjale paradigmu razumijevanja antimikrobnog
učinka i djelovanja na mjestu infekcije, kao i način uzimanja antimikrobnih lijekova. Marketinške kampanje odnosile su se na jednostavnost i kratkoću primjene lijeka te izvrsnu učinkovitost i podnošljivost Sumameda®. U radu su prikazani počeci promocije Sumameda®.

In 1981 a group of researchers at PLIVA created azithromycin, a new antibiotic completely different from the antibiotics in use at the time. In September 1988 PLIVA registered azithromycin as its original antibiotic under the brand name Sumamed®. From the very beginning
the properties of azithromycin indicated that this was a unique antibiotic with excellent efficacy against the most common infectious agents, targeted action at the site of infection, simple dosing, short-term use and excellent tolerability. Pharmacological characteristics and results of clinical trials were used in the original advertising campaigns which resulted in a paradigm shift in the understanding of antimicrobial activity and

2019-nCoV warnings days before officials


First warnings of the deadly coronavirus didn’t come from the Centers for Disease Control and Prevention or the World Health Organization. They came from a robot in Canada.  
Da se radi o epidemiji coronavirusa, umjetna inteligencija kanadskog startupa BlueDot je ustanovila još krajem prosinca prošle godine i poslala "alert" svojim klijentima - uglavnom poslovnim ljudima koji putuju u Kinu, koji su odmah počeli poduzimati mjere zaštite i tako pomogli smanjiti razmjere i brzinu širenja virusa na Zapad.


Tada su bila registrirana samo četiri oboljela od "neobične upale pluća", sva četiri radnika tržnice živim životnjama u Wuhanu. Ipak, tim liječnika, veterinara, epidemiologa, statističara i software developera je napravio takav AI kojem je to bilo dovoljno da prepozna kako se radi o pojavi koja ima potencijal da "eksplodira". Naime, uz službene medicinske podatke, AI Blue Dota je analizirao i profesionalne forume, i vijesti iz medija - o tome da li se netko negdje onesvijestio i slično.

Da je svijet tada alarmiran - ta četiri oboljela bi bilo lako staviti u karantenu, kao i kupce njihovih proizvoda i ostale koji su s njima bili u dodiru, i virus bi danas vjerojatno bio potpuno pod kontrolom.
Na žalost, vlasti država se uglavnom ne pouzdaju u "neprovjerene" usluge malih, inovativnih kompanija, vec čekaju obavijest Svjetske zdravstvene organizacije (WHO) - koja je pomoću klasičnih birokratiziranih procedura, tek tri tjedna kasnije ustanovila da se radi o epidemiji.

BlueDot, an AI-powered health monitoring platform, sent users an alert about the outbreak on Dec. 31. The CDC and WHO reported the news on Jan. 6 and Jan. 9, respectively.
The platform, run by a team of physicians and programmers, uses a sophisticated disease surveillance program to scour news in 65 languages, as well as airline data, to pinpoint issues and alert users to avoid certain areas, like Wuhan, China, where the coronavirus outbreak began.
“We help governments protect their citizens, hospitals protect their staff and patients and businesses protect their employees and customers,” BlueDot’s website reads.

The platform was created by Kamran Khan, the brand’s founder and chief executive officer. In a Wired report, he said after the AI sifts through data, using machine learning to sort out trends and other parallels, epidemiologists ensure that the conclusions and predictions are accurate before alerts are sent to users, government officials and other businesses.
It uses “language processing to train this engine to recognize whether this is an outbreak of anthrax in Mongolia or a reunion of the heavy metal band Anthrax,” he said. “We know that governments may not be relied upon to provide information in a timely fashion,” and “can pick up news, little murmurs or forums or blogs of indications of unusual events.”
To znači da je svijet alarmiran kad je oboljelih bilo već osamdeset puta više nego na dan kad je mali startup odaslao upozorenje svojim korisnicima. Usto, svaki od 312 oboljelih je imao kontakte s ljudima iz obiteljskog kruga, na poslu, u javnom prijevozu i na ulici - pa i sa strancima iz drugih zemalja.
Naime, broj oboljelih nije rastao linearno nego eksponencijalno - dakle dvostruko dulji period da se ustanovi epidemija - tri tjedna potrebna Blu Dotu naspram šest tjedana koliko je trebalo WHO-u -  značio je porast broja oboljelih za čak osamsto posto.
Kao i u slučaju katastrofe u Černobilu, o kojoj su komunističke vlasti šutjele a problem registrirao laboratorij s "demokratskog Zapada" u Švedskoj - i kinesko ministarstvo zdravstva je liječnicima naredilo da slučajeve oboljelih prijavljuju samo interno, sistemu.
Zapad je ovoga puta bio brži, pa je kanadski startup svoje klijente o epidemiji obavijestio dan prije nego su kineske vlasti zabranile informiranje javnosti i uhitile osam liječnika "jer su širili protuzakonite radnje izmišljanja i širenja glasina te uzurpiranja društvenog reda", o čemu je izvijestio Washington Post.
Jasno, širenje panike nije konstruktivno, ali kod brzoširećeg coronavirusa ipak je bilo krucijalno da se javnost informira što prije.
BlueDot je startup čiji tim od četrdeset ljudi ne izgleda impresivno u odnosu na golemu i skupu birokraciju ministarstava država i Svjetske zdravstvene organizacije, i iako nije imao puno klijenata, ali  ako su u početnoj fazi epidemije samo jednog korisnika svoje usluge potaknuli da promijeni ponašanje i tako se sačuva od zaraze, taj mali, jeftini startup je spasio velik broj ljudi. Da su ga vlasti poslušale odmah - danas epidemije možda ne bi niti bilo.

Naravno, svjetski su se mediji raspisali o startup kanadskog Indijca dr Kamrana Khana, zainteresirani za puno jeftiniju i efikasniju uslugu detektiranja epidemija od onoga što nude birokracija i procedure Svjetske zdravstvene organizacije, pa je očekivano porastao interes i za druga rješenja globalne zdravstvene krize koja dolaze iz svijeta inovativnih startupa.
No, zbog nepredvidivog načina i velike brzine širenja coronavirusa - svijet se "zatvara", pa se tako otkazuju i  konferencije u raznim područjima, pa i na temu biotehnologije - dok istovremeno potreba za razmjenom znanja i iskustava i inovativnih pristupa problemu nikad nije bila veća.
Tako je naprimjer otkazana najveća azijska konferencija o zaraznim bolestima u Kuala Lumpuru u Maleziji te je prebačena za jesen, a sudeći prema raspoloženju ljudi koje pomalo zahvaća panika - mogle bi biti otkazane i druge najavljene medicinske i konferencije o biotehnologiji .
Ipak, početkom ovog tjedna je održana koferencija o zaraznim bolestima, farmaceutskoj mikrobiologiji i biotehnologiji u Berlinu, kao i konferencija za investitore Red Herring, i to upravo na temu coronavirusa - koja je u Jeruzalemu prošloga tjedna okupila 23 tisuće gostiju iz 183 zemlje. 
U ožujku je zakazana konferencija o europskom biotechu u Parizu, a početkom travnja farmaceustka konferencija Eye for Pharma u Barceloni. Tu su i konferencije o biotechu u Jeruzalemu u ožujku i Tel Avivu u travnju.
Nadajmo se da će se sva okupljanja održati, dapače da će se hitno organizirati i dodatna, jer unatoč strahu od zaraze i širenja virusa - stručnjaci iz državnih struktura i investitori bi upravo sada trebali intenzivno razmjenjivati iskustva i povezivati se s malim znanstvenim timovima koji su dovoljno fleksibilni da promptno odreagiraju na potrebe tržista vezano uz epidemiju coronavirusa, za razliku od velikih farmaceutskih i biotehnoloških kompanija - koje su zbog inercije velikih sistema spore i "otporne na inovaciju".
Bilo bi poželjno i da birokratizirane organizacije poput WHO-a i ministarstava zdravstva promijene stav prema malim startupima s kojima ne vole poslovati jer su nestabilni i "tko zna da li će postojati za godinu dana" - jer rješenje za obuzdavanje epidemije neće doci iz velikih kompanija, zbog već spomenute "otpornosti na inovaciju".
Kako su za regulaciju i izdavanje dozvola za lijekove i medicinske aparate nužne duge procedure registracije, koje znaju trajati godinama jer uključuju i kliničke studije, u situaciji epidemije virusa koji se ponaša nepredvidivo a brzo se širi najbolje je rješenje potražiti u području koje nije toliko regulirano, jer proizvodi ne predstavljaju rizik za živote ljudi i nemaju nuspojave, poput algoritama za detekciju i dijagnozu, ili  zaštitne odjeće za medicinsko osoblje i građane.
Osim investitora, poticaj za rad malih timova svakako bi bila veća spremnost političkih vlasti da surađuju s "malima", čija je financijska konstrukcija često jedva dovoljna da osigura rad za narednih par mjeseci. Sklapajući ugovore s njima pomogli bi znanstvenim timovima da budu opušteniji i bolje se posvete radu kad znaju da su ekonomski osigurani za naredno razdoblje.

Naprimjer, i europske bi zemlje mogle zatražiti uzorke zaštitnih maski od nano-vlakana, kakve je izraelski startup Sonovia poslao kineskim vlastima koje su odmah počele s testiranjima.
Naime, ako se pokaže da Sonovijina nano-vlakna, koja dokazano sprečavaju prodor bakterija, mogu spriječiti i prodor virusa, pa i corone do tijela osobe koju pokrivaju - ne bi bilo loše da su nadležni već povezani s tom kompanijom, jer će tada nastati neviđena potraznja za njihovim tekstilom i zaštitnim maskama.
Obećavajucim se čini i pamuk obogaćen bakrom s antibakterijskim i antiviralnim svojstvima jeruzalemskog Argaman technologies, originalno razvijen za zaštitu oboljelih od raka, a velika pošiljka njihovog tekstila je poslana u Hong Kong.

Uz rizik izlaganja medicinskog osoblja mikrobima, u jeku velikih epidemija s velikih brojem oboljelih problem predstavlja i nedostatak medicinskog osoblja. U tom smislu obećava projekt umjetne inteligencije - "medicinske sestre superheroja" koji u telavivskoj bolnici Ichilov vodi Ahava Weiss-Meilik.
AI-supersestra moze predvidjeti da će se nekom pacijentu stanje pogoršati satima prije nego sto su to u stanju prepoznati liječnici i medicinske sestre, te predvidjeti da li će se pacijent vratiti u bolnicu nakon što je otpušten.
Pacijentima se na prsa pričvrsti mali senzor poput onoga za EEG, koji je bežičnom vezom povezan s centralnim sistemom. Istovremeno, kamere u bolničkim sobama prate ponašanje pacijenata. Sve te podatke procesuira AI u centralnom sistemu.
- Nema tog doktora, ma koliko god dobar on bio, koji bi tako brzo mogao pregledati sve te podatke i odmah uspostaviti dijagnozu, kaze Weiss-Meilik.
Direktor bolnice Ichilov Roni Gamzu kaze da je AI-supersestra budućnost medicine, a tim u bolničkom I-Medata Centeru  zapravo čini više manjih timova:  AnyVision specijalizira software za prepoznavanje lica, tijela i objekata sa slika te projekt snabdijeva kamerama, BioBeat je razvio maleni bežcni senzor za praćenje vitalnih znakova poput tlaka, pulsa, zasićenosti krvi kisikom, temperature i znojenja, Zebra Medical i Aidoc pomažu radiolozima da na rentgenima i CT-jevima brzo uoče problematična mjesta na pacijentu, dok je startap Agamon specijalizirao procesiranje govornog jezika kako bi iz povijesti bolesti koje su napisali liječnici i medicinske sestre mogli izvući relevantni podaci.
Razvoj umjetne inteligencije tzv. AI-supernurse je omogućilo i to što je zadnjih deset godina povijesti bolesti digitalizirano, a bolnica godišnje ima 400 tisuca hospitalizacija, 1,8 milijuna vanjskih pacijenata, 36 tisuća operacija i 220 tisuća posjeta hitnoj pomoći. Bez te količine podataka projekt ne bio bilo moguće razviti.

Zatim, startup Insilico Medical razvija AI koji bi istraživanje lijekova za pojedinu bolest s nekoliko godina trebao skratiti na nekoliko dana.
U nedjelju je sveučiliste Bar Ilan objavilo da bi tehnologija koju je razvio jedan njihov profesor mogla vrijeme dijagnoze s dva dana skratiti na 15 minuta, a iz kompanije Memed ciji ImmunoXpert - tehnologiju za ustanovljavanje radi li se o bakterijskoj ili virusnoj inflekciji već koriste Švicarska, Europska unija i Izrael - kažu da s kolegama iz cijelog svijeta surađuju na tome da razviju dijagnostiku koja bi bolest otkrila i prije nego se zaraženi osjeti bolesnim.

Coronavirus (COVID-19) and 5G



Many people claims that Wuhan, China, the centre of the new coronavirus outbreak, was where 5G was first rolled out. It suggests that 5G has damaged peoples’ immune systems and so boosted the virulence of the common cold. The main implication of the claim—that 5G can impact immune systems—is totally unfounded. There is no evidence linking the new coronavirus to 5G.

It’s true that Wuhan does have some 5G coverage. 

The local government listed a number of venues with 5G coverage in August 2019. We can’t find evidence it was the very first city with 5G but we’ve seen reports saying Wuhan was one of several Chinese cities where early 5G trials took place. Another said Wuhan was “one of the first pilot cities of the 5G network in China”.

China completes third phase of national 5G trial program

Chinese vendors Huawei and ZTE as well as the country’s three state-owned carriers took part in the trials
China’s IMT-2020 (5G) Promotion Group said it has completed a third phase of non-standalone trials, Chinese press reported.

The tests were based on 3GPP’s Release 15 standards unveiled last year.

Tests in this phase included both indoor and outdoor trials, core network and base station functions. Huawei, ZTE and the China Information and Communication Technologies Group completed trials using the 3.5 GHz and 4.9 GHz frequency bands, according to the reports.

The IMT-2020 Promotion Group said that more than 20 companies participated in the third phase of China’s 5G trials, including Ericsson, Nokia Shanghai Bell, Samsung, Qualcomm, Intel and Rohde & Schwarz.

The group’s next step is to conduct interoperability tests on system and chip system terminals.

China’s 5G R&D tests started in 2016 and have involved three phases: key technologies testing, the verification of technology and solution and 5G system verification.

The IMT-2020 (5G) promotion group was jointly established in 2013 by the Chinese Ministry of Industry and Information Technology, the National Development and Reform Commission, and the Ministry of Science and Technology, based on the original IMT-Advanced Promotion Group. In China, it is the primary platform through which 5G research and international exchange and cooperation is conducted.

Operators participating in the IMT-2020 Promotion Group include China Mobile, China Telecom, China Unicom and Japanese telecoms operator NTT DoCoMo.

The IMT-2020 Promotion Group completed the initial phase of its trial program in 2016. That phase included testing wireless technologies including massive multiple-input-multiple-output, novel multiple access, new waveforms, advance coding, ultra-dense network implementations and high-frequency communications. The trial phase also included network slicing, edge computing and network function reconstruction. The second phase of the national 5G tests were fully completed during last year.

Earlier this year, the Chinese government had authorized carriers to test 5G technology in major cities across the country. Under this initiative, state-run telcos have been deploying 5G networks in 16 cities to trial the technology.

China Mobile, the world’s largest mobile operator in terms of subscribers, had plans to carry out external field test in the cities of Hangzhou, Shanghai, Guangzhou, Suzhou, and Wuhan and deploy more than 100 base stations in these locations. China Mobile also has said it would conduct 5G network application demonstrations in 12 cities including Beijing, Chengdu, and Shenzhen.

Last year, China Mobile announced plans to deploy more than 10,000 5G base stations by 2020. The telco expects to launch a pre-commercial 5G service this year.

China Unicom’s 5G trials plans center around 16 cities including Beijing, Tianjin, Qingdao, Hangzhou, Nanjing, Wuhan, Guiyang, Chengdu, Shenzhen, Fuzhou, Zhengzhou, and Shenyang.

China Telecom has also said it plans to test 5G technology in six cities including Xiong’an, Shenzhen, Shanghai, Suzhou, Chengdu and Lanzhou.

Hubei sets up over 300 5G base station

WUHAN, May 27 (Xinhua) -- Central China's Hubei Province has built more than 300 5G base stations and achieved full 5G signal coverage in its prefecture-level cities, local telecom sources said.The China Mobile's Hubei branch said more than 300 5G base stations have been built since February 2018, and a dozen 5G experience centers have been completed in cities including Xiaogan, Yichang and Jingmen.In the experience centers, visitors can learn knowledge about 5G technology and are allowed to experience 5G networks and high-tech products such as VR glasses, robotic arms and robots. Visitors will also learn about the close connection of 5G products in their daily lives.Wuhan, capital of Hubei, is one of the first pilot cities of the 5G network in China. So far, 5G technology has been applied in various fields in the city, including intelligent education, intelligent medical treatment, driverless vehicles, intelligent tourism, smart transportation and intelligent enterprises.

But it accelerated its plans as tensions with the United States, especially over its boycott of telecoms giant Huawei Technologies, heated up.
“China will have the largest commercial operating 5G network in the world on Friday, and the scale of its network and the price of its 5G services will have a pivotal impact throughout the supply chain,” Bernstein said in a report this week.Authorities have said that they plan to install over 50,000 5G base stations across 50 Chinese cities in the country by the end of this year, and that big cities, including Beijing, Shanghai, Guangzhou, and Hangzhou, are already covered by the 5G network.Chinese companies from Xiaomi to Huawei have also unveiled new products in anticipation of the 5G roll out, with Huawei saying that it anticipates to start seeing a revenue uplift from the sector next year.

As for the claim that the new coronavirus observed in Wuhan is the “normal cold” with boosted virulence—that is simply not the case. 
5G?
Conclusion
Wuhan was one of the first places with 5G trials, as well as several other large Chinese cities, like Beijing, Shanghai and Guangzhou but we don’t know if it was the first.These days 5G is blamed for everything, from cancer to bushfires, so it is not surprising that conspiracy theories that 5G caused, or helped spread coronavirus have been shared into the anti-5G groups.
Some people claimed Wuhan was where 5G was first rolled out, and 5G “wrecked immune systems and thereby boosted the virulency of the normal cold”. Wuhan was one of a few places where 5G was rolled out in China in 2020, along with other parts of the world. There is no evidence (yet) that 5G weakens immune systems or is harmful to humans. Novel coronavirus is probably with us beyond this season, beyond this year.
