Mechanical ventilation a life-or-death choice

In the weeks ahead, physicians may be asked to make decisions that they have never before had to face, and for which many of them will not be prepared. Though some people may denounce triage committees as “death panels,” in fact they would be just the opposite — their goal would be to save the most lives possible in a time of unprecedented crisis. Creation and use of triage committees, informed by experience in the current pandemic can help mitigate the enormous emotional, spiritual, and existential burden to which caregivers may be exposed.

Anticipating the need to allocate ventilators to the patients who are most likely to benefit, clinicians should proactively engage in discussions with patients and families regarding do-not-intubate orders for high-risk subgroups of patients before their health deteriorates. Once patients have already been placed on mechanical ventilation, decisions to withdraw it are especially fraught. Less than 50 years ago, physicians argued that withdrawing a ventilator was an act of killing, prohibited by both law and ethics. Today, withdrawal of ventilatory support is the most common proximate cause of death in ICU patients, and withdrawal of this support at the request of a patient or surrogate is considered an ethical and legal obligation. Withdrawal of a ventilator against the wishes of the patient or surrogate, however, is primarily done only in states and hospitals that permit physicians to unilaterally withdraw life support when treatment is determined to be futile.

Decisions to withdraw ventilators during a pandemic in order to make the resource available to another patient cannot be justified in either of these ways: it is not being done at the request of the patient or surrogate, nor can it be claimed that the treatment is futile. Even though the chances of survival may be low, in the absence of the pandemic the treatment would be continued. Whereas this type of rationing may not be unusual in countries that tragically have a chronic shortage of essential ICU care, it is unprecedented for most physicians who practice in well-resourced countries. Reports from Italy and US describe physicians “weeping in the hospital hallways because of the choices they were going to have to make.

The physicians, nurses, or respiratory therapists who are caring for the patient should not be required to carry out the process of withdrawing mechanical ventilation; they should be supported by a team that is willing to serve in this role and that has skills and expertise in palliative care and emotional support of patients and families. Pain and suffering at the end of life can be controlled, and these patients deserve the best that palliative care can provide.
The decision about initiating or terminating mechanical ventilation is often truly a life-or-death choice.

Growing chestnut and hazelnut from seeds







Hazelnuts and chestnuts, are tasty nuts for baking, cooking and eating out of hand. They can be grown successfully in zones 4 through 9. The hazelnut and chestnut is easier to harvest than some other homegrown nuts, as most varieties grow as a bush instead of a tall tree. Hazel and chestnut trees can easily be grown from hazelnuts  and chestnuts you pick in the fall. Getting the nuts to germinate is a long process designed to imitate a what nature does to each nut, but the results can be a grove full of tree with minimal effort. While most hazelnuts and chestnuts do not reproduce as an exact copy of the parent trees, you will still produce hazel trees with hazelnuts using this method.



Pick hazelnuts and chestnuts about one week after they begin to drop from the tree. The first nuts to drop are often empty shells, so avoid them and wait a week before harvesting. Most hazelnuts and chestnuts drop their nuts early in the autumn, about August or September.



Fill a bucket with water. Remove the husks from the hazelnuts and drop the nuts in the bucket of water. Some nuts will sink and some will float. Remove the floating nuts and discard them. Drain the sunken hazelnuts and chestnuts and package them in a paper lunch bag. Store the bag of nuts in the refrigerator.



Remove the bag of nuts from the refrigerator in November. Fill a bowl with the nuts and cover the nuts with water to soak. Allow the hazelnuts and chestnuts to soak for four days.



Drain the nuts after four days. Mix the nuts with sand or vermiculite, mixing one handful of nuts with one handful of potting materials. Pile the nut mixture in a plastic box or flower pot and place it in the crisper drawer of your refrigerator.



Remove the box from the refrigerator in February, after three months in the cold. Allow the nuts to sit at room temperature for five days. Inspect them after five days to find the nuts that are starting to sprout. Remove the sprouted nuts and place the dormant ones back in the refrigerator for another month.



Fill seedling pots with sterile potting soil and plant the germinated seeds in the pots. Repeat the warming process on the rest of the refrigerated hazelnuts and chestnuts after one month and, if needed, after one more month if there are any left. This will give you two or three batches of seedling trees growing.

Let’s plough Croatia’s fields




The Oikon institute for applied ecology from Zagreb and several experts in agronomy, ecology, forestry and geoinformatics have launched an initiative called “Let’s plough Croatia’s fields”, aimed at urgently boosting the country’s agricultural sector.
Considering that half of the food products in Croatia are imported and the consequences of the coronavirus crisis are only starting to build up, Oikon has decided to use its knowledge and capacity to urgently boost Croatia’s agriculture because Croatia has huge potential, it only needs to know how to utilise that potential, the experts said.
Expert Vladimir Kusan said Oikon experts had conducted a preliminary spatial analysis of potential farmland in Croatia and identified more than 400,000 hectares of idle arable land which could start to be cultivated and used to produce food in a relatively short period of time.
Kusan added that the preliminary maps also include land possibly infested with land mines, however, such land will be eliminated from a new, detailed map that is being made and should be completed in about ten days.
“If it is estimated that one square metre of arable land daily is required per capita, which means that one hectare can feed 27 people an entire year, according to our preliminary calculations, this area of 400,000 hectares could potentially feed almost 11 million people a year,” Hrvoje Kutnjak from the Zagreb Faculty of Agronomy, explained.
That figure is probably significantly lower because a part of the land on the map is not usable and two plots of land may not be equally productive, he said.
“Every crisis is an opportunity. Let’s use this opportunity and mobilise Croatia’s farming potential. Let’s plough Croatia’s fields!,” said Oikon director Dalibor Hatic.


Zaorimo hrvatska polja!


Budući da više od polovine prehrambenih proizvoda u Hrvatskoj dolazi iz uvoza, a kriza uzrokovana koronavirusom tek se zahuktava, odlučili smo upregnuti svoje znanje i kapacitete za hitno pokretanje hrvatske poljoprivrede.
Uslijed teških vremena uzrokovanih pandemijom korona virusa, brojne su države, uključujući i svjetske velesile, prisiljene preispitati svoje politike vezane uz samoodrživost nacije. Pored energije i vode, tu svakako ubrajamo i hranu. Kako bi svojim građanima to osigurale,  neke su države već poduzele mjere kojima će ograničiti izvoz svojih poljoprivrednih proizvoda.
U Hrvatskoj, gdje uvoz potrebnih prehrambenih proizvoda iznosi više od 50 posto, ovo postaje goruća tema u svjetlu očekivanih problema u nadolazećoj ekonomskoj krizi poput smetnji u distribucijskim lancima, nemogućnosti da se zbog društvene distanciranosti poljoprivredni proizvodi prikupe i obrade, problema koji se očekuju u skladištenju proizvoda, itd. Sve su to teme na kojima hrvatsko društvo mora raditi odmah i bez odlaganja kako nam ova poljoprivredna sezona ne bi pobjegla i kako ne bismo imali nestašicu hrane.
Sa željom da u tome pomognemo, odlučili smo upregnuti svoje znanje i kapacitete te učiniti što možemo i što znamo. Okupili smo nekoliko naših odličnih stručnjaka s bogatim iskustvom na području agronomije, ekologije, šumarstva i geoinformatike te zajedno pokrenuli inicijativu pod nazivom „Zaorimo hrvatska polja!“, jer Hrvatska ima velike potencijale, samo ih moramo znati i moći iskoristiti – smatra naš direktor g. Dalibor Hatić.
Dr. sc. Vladimir Kušan, član Uprave Oikona i voditelj dva Oikonovih zavoda, dodaje: – Kao prvi korak napravili smo preliminarnu prostornu analizu potencijala poljoprivrednih površina u Republici Hrvatskoj i identificirali preko 400.000  ha nekorištenog poljoprivrednog zemljišta, koje bi se u razmjerno kratkom roku moglo početi obrađivati ili koristiti za proizvodnje hrane. Nekorišteno je sve ono zemljište koje se može koristiti za poljoprivrednu namjenu, a u zarastanju je ili se ne koristi. U ovim preliminarnim kartiranim površinama nalazi se i minirano zemljište, no sva sumnjiva minska područja ćemo radi sigurnosti izbaciti iz nove detaljne i precizne karte na kojoj već užurbano radimo te očekujemo da će ona kroz desetak dana biti završena.
Neiskorišteni poljoprivredni potencijal u RH prema županijama
Ako procijenimo da je po glavi stanovnika potreban jedan kvadratni metar oranice dnevno, odnosno da jedan hektar može prehraniti 27 osoba kroz cijelu godinu, prema našoj preliminarnoj računici ovom površinom od preko 400.000 ha potencijalno bi se moglo prehraniti gotovo 11 milijuna ljudi godišnje! Ta brojka je naravno značajno manja jer tu moramo uzeti u obzir da dio zemljišta u ovoj karti bez ulaganja nije pogodan za konvencionalnu intenzivnu poljoprivrednu proizvodnju, no taj nam je podatak vrlo dobar pokazatelj da Hrvatska itekako ima mogućnosti i potencijal prehraniti ne samo svoje građane već proizvesti hranu i sirovine za izvoz – objašnjava doc. dr. sc. Hrvoje Kutnjak s Agronomskog fakulteta Sveučilišta u Zagrebu.
Nekorišteno poljoprivredno zemljište u RH prema jedinicama lokalne samouprave
Dr. sc. Zrinka Mesić iz Oikona, ujedno i profesorica na Veleučilištu u Karlovcu, smatra da: – Prema Ustavu  Republike Hrvatske zemljište, biljni i životinjski svijet, odnosno priroda imaju “osobitu zaštitu” Republike. Veliki broj zaraštenih poljoprivrednih površina predstavlja naš neiskorišteni resurs, ali smanjuje i raznolikost našeg biljnog i životinjskog svijeta. Pokretanje obrađivanja neobrađenog poljoprivrednog zemljišta bitno je i za zaštitu biljnog i životinjskog svijeta.
Nekorišteno poljoprivredno zemljište u RH – prostorni raspored
Oikon svoje karte i sve svoje daljnje analize stavlja na raspolaganje svima u Republici Hrvatskoj koji od njih mogu imati koristi. Rezultate svoje analize po pisanom zahtjevu i bez naknade dostavit ćemo u formatu aktivne baze prostornih podataka jedinicama lokalne samouprave, Ministarstvu poljoprivrede, Agenciji za plaćanja u poljoprivredi i ostalim akterima koji su ključni u kreiranju i provedbi poljoprivrednih politika.

CCRES high density orchard







High density orchard is defined as any orchard with more than 150-180 trees per acre. However, many highly productive commercial orchards in Croatia today have 150-180 trees per acre and higher density could be anything over 180 trees per acre. There are several characteristics in addition to tree number that are included in a high density orchard system. Besides having an increased number of trees per acre, a high density orchard must come into bearing within 2-3 years after planting. To achieve this early production, it is essential to use a precocious dwarfing rootstock. Although it is possible to restrict the growth of trees on semi-dwarf rootstocks, they do not have the genetic capacity for early bearing.



High density orchards require trees propagated on dwarfing rootstocks. Presently, only three commercially available rootstock groups or types can be recommended to develop a high density orchard system. Having small trees is not enough; the trees must bear fruit early in the life of the orchard. The rootstocks that are commercially available in Croatia to fit this niche are M.9, Bud.9, and M.26.

Vaše liječenje bolesti COVID -19 trenutno dostupnim lijekovima u Republici Hrvatskoj, ISKLJUČIVO uz suglasnost nadležnog bolničkog povjerenstva za lijekove i Vaš pristanak.

Liječenje se za sada temelji na liječenju simptoma i potpornom liječenju, te mjerama intenzivnog liječenja za teško oboljele. Istraživanja o pokušajima antivirusnog liječenja COVID-19 su malobrojna te za sada ne postoje dokazi o učinkovitom antivirusnom liječenju utemeljeni na kliničkim istraživanjima.

Unatoč tome, prema prvim do sada objavljenim iskustvima pokazalo se u pojedinim skupinama bolesnika da neki antivirusni lijekovi pokazuju određeno pozitivno djelovanje, iako za sada nisu odobreni za primjenu u liječenju COVID-19 te se radi o primjeni lijekova koji nisu registrirani za liječenje COVID-19.

Za liječenje bolesti COVID -19 nužno su u obzir uzete moguće kontraindikacije u primjeni, nuspojave navedenih lijekova (Resochin®) i interakcije koje može imati s drugim primijenjenim lijekovima, kao i eventualna korekcija doze lijekova ovisno o mogućem zatajenju pojedinih organa i organskih sustava.

U slučaju kliničke sumnje ili dokazane pridružene infekcije (influenca (gripa), bakterijska infekcija), ovom obliku liječenja dodaju se i drugi antivirusni/antibakterijski (Sumamed®) lijekovi. Primjena oseltamivira (Tamiflu®) indicirana je samo u slučaju istovremeno dokazane infekcije virusom influence (gripe). Za sada nema dokaza niti indikacije za istovremenu primjenu više lijekova (kombinacije) za liječenje COVID-19.

Resochin®

proizvod koji je Bayer otkrio 1934. predviđen za prevenciju i liječenje malarije, također ima i antivirusna svojstva širokog spektra te dobar učinak na imunološki sustav tijela. Novi podaci iz početnih pretkliničkih i evoluirajućih kliničkih istraživanja provedenih u Kini, iako su ograničeni, pokazuju potencijal za uporabu Resochina u liječenju pacijenata s infekcijom COVID-19.

Resochin® , a product discovered by Bayer in 1934 and indicated for prevention and treatment of malaria, also appears to have broad spectrum antiviral properties and effects on the body’s immune response. New data from initial preclinical and evolving clinical research conducted in China, while limited, shows potential for the use of Resochin®  in treating patients with COVID-19 infection.


Sumamed®

Skupina istraživača-znanstvenika iz PLIVE stvorila je 1981. godine novi antibiotik pod nazivom azitromicin, po svemu različit od ostalih do tada primjenjivanih antibiotika. U rujnu 1988. godine registriran je PLIVIN originalni antibiotik, azitromicin, poznatiji pod zaštićenim imenom
Sumamed®. Osobine azitromicina od samih početaka upućivale su na to da se radi po svemu jedinstvenom antibiotiku koji ima izvrsnu učinkovitost na najčešće uzročnike infekcija, ciljano djelovanje na mjestu infekcije, vrlo se jednostavno i kratko primjenjuje i ima odličnu podnošljivost. Iz farmakoloških osobina i kliničkih istraživanja proizišle su i originalne marketinške kampanje koje su mijenjale paradigmu razumijevanja antimikrobnog
učinka i djelovanja na mjestu infekcije, kao i način uzimanja antimikrobnih lijekova. Marketinške kampanje odnosile su se na jednostavnost i kratkoću primjene lijeka te izvrsnu učinkovitost i podnošljivost Sumameda®. U radu su prikazani počeci promocije Sumameda®.

In 1981 a group of researchers at PLIVA created azithromycin, a new antibiotic completely different from the antibiotics in use at the time. In September 1988 PLIVA registered azithromycin as its original antibiotic under the brand name Sumamed®. From the very beginning
the properties of azithromycin indicated that this was a unique antibiotic with excellent efficacy against the most common infectious agents, targeted action at the site of infection, simple dosing, short-term use and excellent tolerability. Pharmacological characteristics and results of clinical trials were used in the original advertising campaigns which resulted in a paradigm shift in the understanding of antimicrobial activity and
action at the site of infection, as well as of administration of antimicrobial drugs. Advertising campaigns referred to the simple and short-term use as well as excellent efficacy and tolerability of Sumamed®. The paper focuses on the beginnings of promotion of Sumamed®.

The seasonal cycle of respiratory viral diseases has been widely recognized for thousands of years, as annual epidemics of the common cold and influenza disease hit the human population like clockwork in the winter season in temperate regions. Moreover, epidemics caused by viruses such as severe acute respiratory syndrome coronavirus (SARS-CoV) and the newly emerging SARS-CoV-2 occur during the winter months. The mechanisms underlying the seasonal nature of respiratory viral infections have been examined and debated for many years. Studies have revealed the effect of temperature and humidity on respiratory virus stability and transmission rates. More recent research highlights the importance of the environmental factors, especially temperature and humidity, in modulating host intrinsic, innate, and adaptive immune responses to viral infections in the respiratory tract.

Dva glavna faktora su promjene u okolišnim parametrima i ljudskom ponašanju. Studije su otkrile utjecaj temperature i vlažnosti na stabilnost respiratornih virusa i brzinu prijenosa. Novija istraživanja ukazuju na važnost okolišnih čimbenika, posebno temperature i vlažnosti, u moduliranju domaćih, urođenih i adaptivnih imunoloških odgovora na virusne infekcije dišnih putova.

Raise Your Vibration



Have you heard people talking about attaining a higher vibration? Are you unsure what that means or why it's important to you? In this post I am going to address both questions so that you will have a clearer understanding and hopefully want to attain it for yourself and your life.

Becoming aware of your vibration—and the levels of what surrounds you—allows you to seek opportunities to raise your vibration level. Meaning that you can help manifest your own positivity, light, and love. Even if you have no conceptual understanding of what vibrations are, you have certainly felt them. A vibration is a state of being, the atmosphere, or the energetic quality of a person, place, thought, or thing. Everything in the universe is made up of molecules vibrating at different speeds. This includes trees, bodies, rocks, animals, thoughts, and emotions. Human vibrations are composed of everything from physical matter to the way you communicate the thoughts you think. In simple terms, some molecules vibrate faster and some vibrate slower; there are higher vibrations and lower vibrations. When you are vibrating at a higher level, you feel lighter, happier, and more at ease, whereas lower vibrations feel heavy, dark, and confused.

We all vibrate energetically at a particular frequency. The lower the frequency, the denser your energy, and the heavier your problems seem. Here you may experience pain and discomfort in your physical body and experience heavy emotions and mental confusion. Psychically, your energy is darker. You need to exert a great deal of effort to accomplish your goals. Overall, your life takes on a negative quality.

The higher the frequency of your energy or vibration, the lighter you feel in your physical, emotional, and mental bodies. You experience greater personal power, clarity, peace, love, and joy. You have little, if any, discomfort or pain in your physical body, and your emotions are easily dealt with. Your energy is literally full of light! Your life flows with synchronicity, and you manifest what you desire with ease. Overall, your life takes on a positive quality.

Being in a higher vibration is going to become more and more important to you and the rest of the world as we experience greater awareness of the polarities between the lower and higher vibrations. We will begin to perceive greater separation between the "dark" and the "light" almost as if we live in two worlds at once. We will need to consciously choose between the dark and the light or the higher and the lower vibrations.

Despair and desperation due to challenges will consume those vibrating at a lower frequency. Know that help is available from those vibrating at the higher frequency. The lower and denser energies will naturally want to rise when around the higher and lighter energies. However, this process will feel very uncomfortable for people who carry the lower energies and people who carry the higher energies for a while. It is important for people who carry a higher vibration to maintain it and not lower it no matter how uncomfortable it feels. Those people resonating at a higher vibration will only be able to maintain it as long as they don't succumb to the pull of the lower vibration. That pull will come in the form of old programs and patterns of needing approval or of wanting to "fit in."

When you choose to be in a higher vibration, you are an example. Choosing the light of a higher vibration requires a firm commitment to it even when it feels as though you no longer fit in. You won't receive approval or fit in any longer because you WILL stand out from those with a lower vibration just as a tiny light in the midst of a dark room stands out. Understand that those with the lower vibration have the potential to raise their frequency. Remember that you will be able to find people who resonate at a higher vibration where you will have the support you need to keep your light shining bright.

Pathways beyond Sustainability


There are 12 design principles in permaculture. I think they can be applied to daily life, work and play which will allow us to live happier and healthier, to work more effectively and to create a more sustainable world.



Here are the 12 principles:
Observe and interact.
Catch and store energy.
Obtain a yield.
Apply self-regulation and accept feedback.
Use and value renewable resources and services.
Produce no waste.
Design from patterns to details.
Integrate rather than segregate.
Use small and slow solutions.
Use and value diversity.
Use edges and value the marginal.
Creatively use and respond to change.
How can we use these principles to guide our life or our work?

Observe and interact- Let’s slow down and observe and appreciate our surroundings and others. Let’s slow down and interact with others, in real life, not on our phones.
Catch and store energy- Take time to relax to restore balance and energy. Also, for me this means associating myself with those who bring positive energy and it means doing things that bring me joy and make me feel energized like running and learning.
Obtain a yield- In permaculture, this of course might mean planting in such a way that the most food is created. The food is the reward. In life, we can work to obtain an intrinsic or extrinsic reward. These rewards need not be strictly financial. Perhaps the reward is making a difference in your client’s life or making your coworker’s day a little bit easier.
Apply self-regulation and accept feedback- Self-regulation or self-control allows us to be accountable and it empowers us. Feedback helps us improve which will help us excel at work.
Use and value renewable resources and services and produce no waste- These two principles are simply common sense tips to living a eco-friendly lifestyle and in creating a more sustainable world.
Design from patterns to details- Permaculture calls for standing back and observing patterns in nature and in society and then filling in the details; this might make for a more efficient and logical way to work as well.
Integrate rather than segregate and use and value diversity- These principles translate perfectly in how we should all live in harmony with each other by integrating everyone into our communities and valuing diversity.
Use small and slow solutions- Permaculture uses small and slow solutions because they are easier to manage than bigger solutions and they allow for the use of local resources leading to more sustainable outcomes. While this may not always be feasible- sometimes we need big and fast solutions to solve some of society’s greatest problems- this principle can potentially be a valuable tool for problem solving.
Use edges and value the marginal- Permaculture suggests that the interface between things is where the most interesting things take place. These are often the most valuable, diverse and productive parts of the system. This might mean extending a community garden out onto an edge near an unused railway. In life and work, we can try to more effectively use our resources including the edges.
Creatively use and respond to change- This requires no explanation, but in a changing world, it remains an important principle in both life and work. To remain a viable and sustainable business and to live well, we must be resilient and creatively work with change.

Using these principles may allow us to create a better life and a better world by caring for the earth and caring for people.
